How long will it last?
The evaluation will be 1.5 2 hours total.
What do I need to bring?
#1. Evaluation Paperwork (from this website) Including the 2 HIPAA forms signed
#2. Previous testing by a psychologist
#3. Previous testing by the school
#4. Copy of the divorce decree, if you are divorced.
#5. Previous report card
#6. Disciplinary forms (if any)
#7. Copy of records from previous providers
What if I don't have everything before the evaluation?
If you don’t have all the materials you need, that is ok.
However, you must have the evaluation paperwork completed. Having all of the requested materials just allows for a more thorough evaluation.
I am nervous, what is it like at the Center?
People are often anxious about going to a counselor’s office. At the Center, we have a relaxed atmosphere. The lobby and main office look like a living room (see pictures under the location tab). We will just talk for a while and the counselor will provide you with suggestions about how to make things better.
What will we do at the evaluation? Here is an outline:
Welcome!
Please ring the bell when you arrive.
- The counselor will greet you and introduce herself. She will answer any questions you may have before the evaluation begins and explain the process of the evaluation.
- Next, please provide the completed evaluation paperwork, which you printed off of this website, and the other requested documents (see above).
- The counselor will take some time to thoroughly review the paperwork. During this time you will be asked to review a packet about office policy and sign a consent form allowing the counselor to evaluate your child. (Don’t worry, that’s the last of the paperwork!)
- When the counselor has finished the review of paperwork, she will interview your child individually.
- When the counselor has completed the interview with your child, she will meet with you privately. Please feel free express concerns and ask questions. Questions are always welcome!
Does attending the evaluation mean I am signed up?
At the end of the evaluation, you and your child will join together with the therapist who will provide specific treatment recommendations. If further treatment is recommended, you can decide if you want to return.
The evaluation is just that, an evaluation only. It is a great opportunity to see if your family and the therapist are a good fit.
The treatment recommendations will be specific and will clearly state what the therapist believes can improve things with your family. If the therapist does not recommend further treatment, she will tell you that as well.
What kind of therapy is this?
The therapy offered is brief and solution focused. It is different from traditional supportive therapy because we won’t just talk about the issue. We will develop action plans or do skills training to compensate for weak areas. We may need to do social skills training, in which specific social lessons are presented and practiced. We may need to develop an anger coping plan. We might need to develop a plan to decrease anxiety or depression. We might need to develop systems to get organized for school or to obtain more feedback from the school. The therapy is action-oriented.
What if my child has a severe issue?
Children who have a pervasive issue such as Aspergers, Autism, PDD, or chronic depression, the treatment time may need to be extended. Action plans are still developed to address the issues directly, yet the interventions may require more than 8-10 sessions.
Do I have to take notes?
You will leave the evaluation with:
- Written treatment interventions
- A list of local and online resources including psychiatrists, if applicable.
- A handout that talks about how to get the most out of our time together in subsequent sessions.
- A copy of the consent you signed in the lobby.
- Highlights of the consent form.
- A thorough receipt you can submit to the insurance company for reimbursement.
How can I pay?
The entire evaluation process can last up to 2 hours (a double session and a half). The evaluation rate is $175.00. Check, cash, Mastercard and Visa are accepted.
Do you accept my Insurance?
Audra Boxma is not on any insurance panels, thus she is not an "in network provider". If you have an HMO, she will not be covered. However, if you have a PPO or POS plan, then you likely have out of network benefits. Audra Boxma is frequently covered under out of network benefits. You may have an out of network deductible to meet, then the insurance company reimburses you directly at your out of network rate. It may be 50-80% reimbursement depending on your plan.
Each insurance plan is different, thus it is important to call your insurenace company directly for clarification. Also, ask if there are additional criteria which must be met (such as a specific diagnosis) in order to receive reimbursement. Last, ask about the type of therapy benefits you have. The most common codes are:
90806 Individual Therapy
90847 Family Therapy
90846 Family Therapy without patient (parent only meeting)

You are under no obligation to return for therapy; and once initiated, you have the right to terminate at any time. However, it is important to discuss if everyone is willing to make the commitment. To obtain the best chance of success, it is important that you all make a commitment to attend sessions as recommended and follow through with treatment until all of your goals are completed.
